CHS was established as a new division of New York City Health And Hospitals in part to bring greater transparency accountability to the provision of healthcare in New York City jails after decades of contracted healthcare providers most recently Horizon a for profit correctional healthcare company.

As a new division of our municipal healthcare system, CHS adopt and expanded a clinical review process for each death, structure around well established, and universally accepted quality assurance principles and protocols in healthcare.

This multi step rigorous approach which begins immediately after every patient death includes video and record review, discussions with relevant staff members, and a focused case review with CHS clinical leadership.

The review process identifies area for improvement and wherever appropriate, corrective action plans to improve care quality even when not directly related to the cause of death.

This process precedes the additional supplemental death reviews conducted by the New York City Health And Hospitals Board, the New York City Board of Correction, and the State Commission of Correction, otherwise known as COC.

When applicable, CHS's human resources and clinical leadership ensure staff involved in an adverse clinical event received counseling, training, and education, and when warranted discipline.

This occurs in consultation with CHS health CHS's health care unions, the New York State Nurse Association, DC 371199, and doctors counsel to ensure our doctors, nurses, and other healthcare staff receive appropriate professional development as well as legal protections.

In accordance with Apple laws, CHS does provide BOC and other authorized agencies and Oversight Boards, including SAOC, The medical records of a person who dies in DOC custody and works with DOC as they review the case and write their public report.

CHS also provides, with patient consent, complete and select medical records to defense attorneys on request.

CHS established a productive working relationship with BOC in conducting thorough reviews of the draft reports.
