This chapter discusses the potential impact of proposed federal budget cuts on older adults in New York City, particularly focusing on Medicaid and long-term care services. Commissioner Lorraine Cortés-Vázquez explains that while not under DFTA's purview, these cuts would significantly affect older adults receiving medical and in-home services, especially those with the greatest needs and lowest incomes.
- The proposed $880 billion cut in federal spending over ten years threatens funding for Medicaid programs
- Long-term managed care, which provides the bulk of in-home services for older adults, would be severely impacted
- DFTA is preparing scenarios based on potential cuts to federal funding for services like case management, meals, transportation, and elder abuse prevention

Thank you, chair and thank you commissioner.
3:56:25
a question about the federal government.
3:56:28
They're considering a cut of $880,000,000,000 in spending in the next ten years.
3:56:35
Move that threatens funding for Medicare programs or Medicaid programs rather.
3:56:40
So what I wanted to ask was how would this affect older adults in New York City?
3:57:30
And I know you said every every
3:57:55
Is it possible also in that answer for there to be maybe a zeroing in on the demographics that might be more affected than others?
3:58:22
Is there a plan to address a budgetary deficit that might arise from something like that?
3:58:49
Gotcha.
3:59:10
Well, thank you.
Rafael Salamanca, Jr.
3:56:23
Hi.
3:56:24
Have
Lorraine Cortés-Vázquez
3:56:49
It would, it's not under the purview of the Department for the Aging, of NYC Aging.
3:56:55
But what I can tell you is that every long term care older adults, every older adult receiving long term care will be affected.
3:57:06
Every older adult receiving other medical services will be affected.
3:57:13
The impact will be great on the older adult population.
3:57:20
That coupled with the social security conversations also presents a threat for the older adult population.
3:57:32
And just know that long term managed care is the bulk of the in home services that older adults receive because the ISEP services are just a small portion.
3:57:45
Those state non Medicaid home care services are a small portion of all of the services that older adults receive in the home.
3:58:09
Yeah.
3:58:09
Because those with the greatest needs and the lowest income are the those that are gonna be the affected.
3:58:15
That's who the Medicaid client is and that's the Medicaid client receiving essential in home services.
3:58:32
Well Medicaid is not under our purview.
3:58:35
But what we have been looking at is some scenarios based on the federal funding that NYC Aging receives.
3:58:43
But it is not around Medicaid or long term managed care.
3:58:50
It would affect as I said earlier, to Chair Brennan as well as to Chair Hudson.
3:58:56
It'll center around case management, home delivered meals, congregate meals, and things of that nature.
3:59:05
And transportation.
3:59:06
And elder abuse.
