Council Member Hudson asks about changes in the Community Care Plan's goals and improvements in service integration. Commissioner Cortés-Vázquez explains that the plan has evolved rather than changed:
- The plan has adapted to improve service delivery, such as incorporating geriatric mental health services into more older adult centers
- Integration and collaboration between older adult centers, NORCs, and other community resources are encouraged through partnerships
Additional details:
- The hub and spoke model was implemented for geriatric mental health services to broaden their reach
- The plan emphasizes collaboration at local levels and across agencies
- Efforts are being made to secure additional funding sources through partnerships in government

Did you do you think the goals of the plan have changed since it's been announced.
Lorraine Cortés-Vázquez
0:30:32
I don't know that it's changed as much as it evolves.
0:30:36
So if change is if evolution is part of change, yeah.
0:30:42
When we think about our geriatric mental health services.
0:30:46
Right?
0:30:47
We always had those.
0:30:48
But as part of the community care plan, rather than having independent geriatric mental health centers, we incorporated that and came up with that hub and spoke.
0:30:58
So that more older adult centers would have geriatric outlets.
0:31:02
So geriatric cancer health services.
0:31:04
So, you know, so it's been involved.
0:31:06
It's not have we added new services?
0:31:08
No.
0:31:09
We've looked at the implementation of those services and always that we can have the broadest reach.
Crystal Hudson
0:31:14
Okay.
0:31:14
Great.
0:31:16
Can you describe the different ways in which the plan in which the plan has been implemented since its inception in in 2021 specifically with regards to improving integration and collaboration between older adult centers, naturally occurring retirement communities and other community resources.
Lorraine Cortés-Vázquez
0:31:44
Other than the constant encouragement of that and the partnerships on the ground, I would say that the Plan forces that encouragement, and it continues.
0:31:57
Am I understanding the question correctly?
0:32:14
Chair, is that an answer that you're seeking at the more partnerships at the local level?
Crystal Hudson
0:32:22
Sorry.
0:32:22
I I didn't hear you.
0:32:26
I think whatever is most relevant.
Lorraine Cortés-Vázquez
0:32:32
Yeah.
0:32:33
And I thought, yeah, at at the local level, we always think the strength of this plan goes with it really is about collaboration, you know, locally, but it is also collaboration across agencies, which we're doing with the cabinet, and and also some of our partners in government, meaning other funding sources.
0:32:52
Which is something that we're constantly trying to advocate for.
Crystal Hudson
0:32:55
Okay.
