CHS has established multiple pathways for patients to report sexual assault, abuse, and harassment. These mechanisms ensure that patients have various options to disclose abuse and seek help.
- Patients can report abuse to any DOC staff member
- Disclosure can be made to any CHS staff during any clinical encounter
- Patients can call the CHS health triage line to speak directly with a nurse
- Family members and external parties can contact CHS's patient relations department
- A 24/7 CHS operations phone line is available for reporting concerns

In addition to initiating a report with any DOC staff, a patient can disclose abuse to any CHS staff person during any clinical any encounter, including another clinic appointment, or can call the CHS health triage line to speak directly with the nurse.
