When an individual is determined eligible for APS services, a caseworker develops a comprehensive service plan tailored to their needs. This plan can include a wide range of interventions and assistance.
- Services may include medical and psychiatric referrals, assistance with benefits applications, and eviction prevention
- Financial management, heavy-duty cleaning, and referrals to law enforcement for abuse cases are also available
- In some cases, APS may petition for community guardians to manage property and personal affairs

When an individual is determined eligible for APS services, the caseworker develops the service plan that can include any of the following, a referral for psychiatric and or medical examination and ongoing care, assistance in obtaining a recertified Medicaid and home care, applications for payment of rental and utility arrears, assistance in obtaining public assistance benefits and obtaining recertifying supplemental security income, SSI or social security disability SSD benefits, petitioning the housing court for a guardian ad litem to assist with eviction prevention, identification of alternative living arrangements, financial management of social security benefits, referrals to the NYPD and district attorney to address allegations of exploitation and abuse, heavy duty cleaning services, and petitioning the supreme court for community guardians to manage property and personal affairs.
